The Fuel Poverty Project
The Primary Care Network (PCN) has collaborated with 361Energy CIC to identify patients in our region who are experiencing health issues related to: atrial fibrillation (AF), coronary heart disease (CHD), chronic kidney disease (CKD), heart failure (HF), stroke/transient ischemic attack (TIA), and childhood asthma. By utilizing postcode data, 361Energy will assess the risk of fuel poverty among these individuals, enabling targeted support for those in need.
Supporting the people of North Devon & Torridge—providing free energy saving advice to vulnerable households, plus Retrofit Coordinators who can advise on suitable ways to upgrade homes to run more energy efficient.
The vision - to eradicate fuel poverty and redress the health inequalities from living in low energy efficient housing alongside a sustainable future.

Livestock market health inequalities project
One of our primary responsibilities is to tackle health inequalities directly focusing on prevention, and more specifically addressing Core20Plus5 targeted interventions. We identified a missing link between those working within the farming industry and a connection with their health and we sought to create an innovative way to bridge this gap.
The PCN initiated monthly health checks at Holsworthy Livestock market as part of the Health Inequalities Project. This initiative is ideally suited for our local community and also aims to enhance the connection between the PCN and local farmers and their families. We chose the Holsworthy Livestock Market as a venue to engage with the farming community.
Looking to expand the successful pilot it was important to us that we understood the specific needs and wants of those working within the agricultural community and have been fortunate enough to collaborate with Exeter University to drive research and expand delivering healthcare at your door.
We were successful in a grant bid which enabled us to receive funding from Cornwall ICB which enabled us to support two roles: a community navigator through IMAGINE IF and a nurse/HWC through the PCN - Debbie Gardener. A new Nurse Associate - Elizabeth Flannagan joins our livestock team from early January 2026.
Our award winning Farmer's Health Inequality project
We are immensely proud that our Farmer's Health Inequality project was shortlisted for the Integrated Personalised Care Award
This award recognizes exceptional personalized care initiatives that empower individuals receiving healthcare, as well as their families and support staff throughout the South West.
We are delighted to announce that we were successful in winning the award at the ceremony in Somerset.
